COVID-19 VACCINE SCHEDULING AVAILABLE FOR AGES 5 AND UP

Atlantic Health System has available vaccine appointments at local health facilities near you.  They offer online scheduling for eligible individuals to receive their first dose, third dose or their booster of COVID-19 vaccine.

*Moderna, Pfizer-BioNTech and Johnson & Johnson vaccines available.

Please answer a series of questions, by visiting the COVID-19 Vaccine Appointment Page, regarding the patient's eligibility, pick a date and time and provide the patient's information. Vaccines are available at no cost to you.
